Common antihypertensive dosages

Angiotensin-Converting Enzyme Inhibitors

Note: consider discontinuation or dose reduction of diuretic when initiating an ACE inhibitor to avoid potential hypotension

  • Perindopril (Coversyl)
    • 4-8 mg PO daily
  • Ramipril (Altace)
    • 2.5-20 mg PO daily; may consider divided dosing (BID) if necessary to maintain BP
  • Lisinopril 
    • 5-40 mg PO daily (usually start at 10 mg if not on diuretic)

 

Angiotensin II Receptor Blockers

  • Olmesartan (Olmetec)
    • 20-40 mg PO daily
  • Candesartan (Atacand)
    • 8-32 mg PO daily (may be divided over two doses)

 

Thiazide Diuretic

  • Hydrochlorothiazide
    • 12.5-50 mg PO daily (may be divided over two doses)

 

Thiazide-like Diuretic

  • Metolazone (Zaroxolyn)
    • 2.5-5 mg PO daily

 

Loop Diuretic

  • Furosemide (Lasix)
    • 10-40 mg PO BID (e.g. 8 am & 2 pm; titrate per patient)

 

Potassium-sparing Diuretic

  • Spironolactone (Aldactone)
    • ​25-100 mg PO daily (may be divided over two doses)

 

Beta-blockers

  • Metoprolol (Lopressor, Toprol XL)
    • 50-450 mg PO daily (immediate release should be divided over two-three doses)
  • Atenolol
    • 25-100 mg PO daily (start 25-50 mg daily)
  • Bisoprolol (Zebeta)
    • ​2.5-20 mg PO daily (start 2.5-5 mg daily)

 

Calcium channel blockers

  • Dihydropyridine
    • Nifedipine (Adalat XL)
      • 30-90 mg PO daily
    • Amlodipine (Norvasc)
      • 5-10 mg PO daily
  • Non-dihydropyridine
    • Verapamil
      • 80-160 mg PO TID (immediate release)
    • Diltiazem (Cardizem, Tiazac)
      • 180-480 mg PO daily (Cardizem CD) (start 180-240 mg daily)
      • 120-540 mg PO daily (Tiazac, Taztia XT) (start 120-240 mg daily)
      • 180-360 mg PO daily (Cardizem LA, Tiazac XC) (start 180-240 mg daily)
