Early Mild Small Bowel Obstruction

  • Otherwise well person with a high protein diet (or some other likely trigger)
  • Minimal (e.g. 1/10) pain
  • No nausea (or very minimal nausea)
  • Few small bowel air-fluid levels on abdominal series
    • Usually stool in region of transverse (and distal) colon
  • May have mild leukocytosis but normal CRP

Management

  • Reasonable to give a trial of fluid diet and suppositories plus enema prn to see if it will resolve
  • Strict instructions to return to ER if symptoms do not resolve, or worsen, or new symptoms develop
